Clinical Role Overview
East London NHS Foundation Trust is seeking a Bengali/Sylheti speaking dementia care advisor/navigator in Tower Hamlets. The role provides post‑diagnostic information and signposts services to support people with dementia and their families, as part of the Integrated Care Mental Health team.
You will visit clients at home, contribute to personalised care plans, and work with carers and local services to adapt care as the illness progresses.
