Community Mental Health Nurse
Clinical Role Overview
This role is focused on improving outcomes for people living with severe and enduring mental health difficulties. You will work collaboratively with service users, their families, and carers to co-produce personalised care and risk management plans that promote recovery, enhance wellbeing, and support individuals to live as independently as possible. Success in this role relies on building and maintaining effective partnerships with a wide range of services, including GPs, Crisis Resolution and Home Treatment Teams, housing providers, social care, and voluntary and community sector organisations. You will take a proactive and creative approach to ensuring people receive the right care, at the right time, from the most appropriate service. Where required, you will facilitate access to specialist services, including Early Intervention in Psychosis and Eating Disorder Services. As a valued member of the multidisciplinary team, you will:
Key responsibilities
- Undertake comprehensive assessments and contribute to the development, implementation, and review of recovery-focused care plans.
- Actively participate in multidisciplinary discussions, formulation, risk management, and care planning.
- Manage a caseload and work collaboratively with service users, families, and carers to achieve positive outcomes.
- Contribute to service development, quality improvement initiatives, and the implementation of evidence-based practice.
- Participate in regular clinical, professional, and managerial supervision.
- Maintain high standards of professional practice, working in accordance with Trust policies, professional standards, and ethical frameworks.
- Support individuals to access local health, social care, and community resources that promote recovery and inclusion.
- Ensure accurate and timely record keeping in line with Trust standards, national guidance, and NICE recommendations.
What we offer
- Regular management, clinical, and professional supervision.
- Access to a wide range of training, learning, and continuing professional development opportunities.
- A supportive and experienced multidisciplinary team environment.
- Opportunities to contribute to service innovation, quality improvement, and the ongoing transformation of community mental health services.
We are seeking an enthusiastic, compassionate, and motivated practitioner with excellent communication and interpersonal skills, strong clinical knowledge, and a commitment to delivering high-quality care in the community. You will be flexible, resilient, and committed to recovery-focused, person-centred care, with the ability to work effectively as part of a multidisciplinary team.
