Clinical Role Overview
Southend University Hospital NHS Trust is seeking an IDT Nurse Assessor to join the Integrated Discharge Team at Broomfield Hospital on a maternity leave cover until August 2027. You will assess patients for discharge, identify ongoing care needs and coordinate timely referrals to community and system partners to ensure safe, effective and patient-centred discharge.
