Clinical Role Overview
West Hill Health is recruiting one Clinical Pharmacist to join our PCN team in central London. The role emphasizes chronic disease management, diabetes reviews, and medicines reconciliation within a multidisciplinary setting.
You will work with doctors, nurses, HCAs and pharmacists, with CPPE funded prescribing courses and possible Independent Prescribing training. Based in one practice, you will contribute to high-quality primary care.
