Clinical Role Overview
Nottinghamshire Healthcare NHS Foundation Trust requires a Ward Carer Link and social visitor accreditation lead to support the Triangle of Care (TOC) agenda, organise 1:1 carer support, and coordinate visitor meetings and interpreters. You will liaise with social workers, ward staff and MDT colleagues, and use the RIO system to maintain patient and carer records.
The role emphasises safeguarding, staff training, and improving patient and carer experiences.
