Social Prescribing Link Worker — PCN Health Navigator
Clinical Role Overview
NHS Jobs is seeking a Social Prescribing Link Worker to promote social prescribing, support self-management, and address health inequalities. As part of a PCN multi-disciplinary team, you will build relationships with GP practices, attend MDT meetings, and provide information and feedback to enable appropriate referrals.
You will meet people one-to-one, perform holistic needs assessments using PAM and the ONS4 Wellbeing questionnaire (full training provided), and help identify wider issues
